The Carer Friendly GP scheme is a new initiative being operated by Telford & Wrekin Carers Centre. It is an agreement that forges a closer working partnership between a GP surgery and the Carers Centre, with the aim of increasing carer identification and support. As part of the scheme, each practice will display an information board for carers and have a folder containing relevant information and support for carers. They will also provide registration forms for people to identify themselves to the practice as a carer.
A key part of the agreement is that the practice will appoint a Carers Champion who will have an enhanced knowledge of signposting and supporting carers within the surgery and will be be the key link worker with the Carers Centre. View details of the GP Carers Champion Role.
